Set Default Review Playbook

POST /review-playbooks/{playbook_id}/set-default

Promote it, demoting whatever held the flag at the same scope.

One default per scope is three partial unique indexes in the schema, so the demotion is not a courtesy — without it the promotion is an IntegrityError.

Role floor

Requires admin or above. The floor is a minimum, not a grant: scope filtering still applies on top of it, and a resource in another tenant answers 404 rather than 403.
