Reset Knowledge Base Structure

POST /knowledge-bases/{kb_id}/reset-structure

Delete hubs and proposed edges; keep items, confirmed edges and rejected ones.

A rejected edge is a tombstone (RULE KE-1). Sweeping it would turn “an operator refused this topology” into “the organize stage may try again”, which is the one thing a reset must not undo (D-739).
